Geriatric psychiatrist Dr. Robert Kohn treats older patients with different intellectual and developmental disorders. Robert Kohn, MD has traveled to many countries as a researcher, scholarship grantee, and temporary advisor for the World Health Organization. Most of his studies abroad focused on psychiatric epidemiology, cultural psychiatry, and global mental health. Dr. Robert Kohn maintains his membership in the American Psychiatric Association (APA) where he is also a distinguished Fellow. APA is a premier psychiatric association established to promote equitable access to the best available healthcare for all people with mental disorders and substance disorders. To further the rights and interests of patients in this regard, APA created the Presidential Task Force on Assessment of Psychiatric Bed Needs in the US in 2020 to assess the country’s growing psychiatric bed crisis. In 2022, the Task Force completed its mission. Their report, entitled “Psychiatric Bed Acess in the US: Understanding the Problem and Moving Toward Solution,” was presented at the APA 2022 Annual Meeting in May. The report said that access to psychiatric beds has significantly declined in the past six decades. There is a notable absence of effective means, even at the community level, to measure how many beds are needed to meet the demands. Because of the lack of psychiatric inpatient beds, most patients are boarding in emergency rooms if not discharged prematurely. The social impacts of this crisis include homelessness and even incarceration. To solve the problem, the Task Force proposes a new model for estimating the needs, including a means to inform community leaders and policymakers about appropriate capacity build-up. The model specified separate approaches for children and adolescents and adults. The model applies even to hypothetical “Anytown, US.” The University of Michigan is the first to test the new model in its community. Led by the University’s Psychiatry Department, data are now being collected to be fed into the APA model. In general, the treatment gap refers to the public health indicator that highlights the unmet healthcare needs. It is the absolute difference between the number of people with a particular disorder and the number of individuals with that condition who receive appropriate medical attention. In the technical document entitled “Treatment Gap in the Americas” prepared by Robert Kohn MD for the PAHO, the reasons these treatment gaps remain prevalent in the Americas were outlined. Governments have generally failed to enact laws that protect people with mental illnesses, ensure parity to mental disorders with other diseases, provide sufficient funds for mental healthcare, and specify the type of healthcare services provided to such patients. The structure of the mental health system, such as the availability of psychiatry beds and outpatient services in general hospitals, have bar care for mental patients. Governments should also make mental healthcare services accessible to people in urban areas instead of only concentrating in large urban areas. Individuals' beliefs and attitudes on mental health have contributed to the growing treatment gap. The psychiatric epidemiological studies conducted among adults in Chile, Canada, and the US shed light on this. The respondents believe that they would get better and that they can handle the treatment on their own. Some reported inconvenience in seeking professional treatment, and others felt their healthcare providers cannot be trusted. Others said they are financially unable to support treatment.  A Distinguished Fellow of the American Psychiatric Association, Dr. Robert Kohn received his MD at the University of Illinois Chicago and went on to complete his psychiatry residency at Brown University. Over the last five years, Dr. Robert Kohn has operated his own private practice in Providence, Rhode Island, which focuses on geriatric psychiatry.
Geriatric psychiatry is a branch of medicine that focuses on the evaluation, diagnosis, treatment, and prevention of mental conditions in seniors. Geriatric psychiatrists also research the psychological aspects of aging. According to an article on the National Institutes of Health (NIH) website, three commonly-diagnosed psychological syndromes among seniors are depression, dementia, and delirium. Also called major depressive disorder, depression is characterized by a perpetual loss of interest in life and persistent sadness. Dementia can result in communication problems, loss of memory, and decimated problem-solving skills. People who are living with delirium have difficulty thinking clearly and are often disconnected from the environment. The aforementioned syndromes are sparsely connected. Dementia is sometimes triggered by delirium. A senior may have both depression and delirium at the same time. Symptoms of delirium, depression, and dementia sometimes coexist. To manage symptoms of any of the conditions and minimize the risk of developing the consequential syndromes, diagnosis and timely treatment are crucial. |
